Please fill out the external form with the relevant contact info of the individual(s) requesting or needing access. What type of report will be needed? Who will be using this report? Depending on the type of information requested there may be approval required from SCCMHA Management.
Sentri Add or Remove Staff Form
Provider request for new reports or report modification
used to request an app that has not yet been previously approved for DME use.
Add/Remove Guest Access to Teams
Provider Sentri account activation, and password change
Existing accounts only
If the correct request is not available above or the correct request does not exist please submit a Provider General Support form
Any Provider General Support requests that have an existing service category above will be cancelled
