What Is It?

Sentri Add/Remove Staff form for external providers.

Who Is Eligible to Use It?

External providers who need staff to access or be removed from SCCMHA EHR (Sentri) 

How Do I Use It?

By filling out this form, the SCCMHA auditing department will review and approve or deny EHR (Sentri) access. Once approved by the auditing department, an SCCMHA IT staff will create the Sentri account and send it to the requesting supervisor. If the request is denied the request will be cancelled with no action.
