External Training Documentation Form

Use this form to submit external training documentation to SCCMHA Continuing Education. This will help guide the entry of your training credit(s) to assure accuracy.  You will typically see the training credit on your next quarterly transcript report. 

User Instructions & Tips

REMEMBER:  

•  Do Not Resubmit SCCMHA Training Certificates.
•  For Case Manager Core Elements - attach your tracker with completion dates along with training transcript or certificates.
• Allow up to 30 days for this request to be processed.

 

Upload your certificate or proof of completion that includes training date(s) and length.

Also include documentation about the training topic. This is necessary to justify Child Diagnostic Contact hours if the training title doesn't reference relevant key words.

This is not necassary for Suicide Trainings Credit.

If you are requesting Social Work, MCBAP, or SUD Contact Hours - the certificate must state the training qualifies for these credits.

Relias Transcripts should only be submitted if you are requesting documentation of multiple Child Diagnostic Contact Hour trainings, and the training title identifies a key word related to Child Diagnostics.

If you are uploading more than one document, you have two options:

1) Scan documents as one .pdf file and upload the one file.
OR
2) Select Multiple files to upload by holding the "Ctrl" key when clicking on each file to select them.
